Mater Dei Parish offers these tools and resources to assist you and your family in discussing your wants, beliefs, and wishes. They will aid your loved ones in fulfilling their responsibilities in love on your behalf.
1 - Many do not realize there is a Catholic Healthcare Directive. You can articulate your beliefs and values about your healthcare.
Complete the "fillable form" or download it. The document must be signed or notarized. Please keep a copy for yourself. Your healthcare agent and providers should also have a copy to ensure your requests are honored.
2 - Finishing Life Faithfully - A Guide for Catholics on End-of-Life Care was created by Aging with Dignity in 2022.
Discussions include healthcare decision making, aggressive treatment, minimal consciousness, managing pain, "Do Not Resuscitate," Five Wishes, and Physician Orders for Life-Sustaining Treatment (POLST).
3 - The Catholic Funeral Planning Guide is an in-depth document which allows you to prepare, guide, and supply your loved ones with the necessary information to carry out your intentions for your funeral planning and surrounding rites.

c. Contact the office. We will print it for you and have it ready for you to pick up.
Once completed, please keep a copy of the guide for yourself and provide a copy to your loved ones and/or funeral home.
4 - Leave a Legacy of Faith and Charity is a simple, easy-to-follow guide to help you get your estate in order, regardless of the size of your estate.
Complete the "fillable forms" or download the booklet. It is a great resource to help you and/or your attorney prepare your will.
